Should parents clean under a boy's foreskin?

No. The foreskin is stuck to the head of the penis at birth and parents should not try to pull it back during infancy or to wash under it. A yellow material often builds up underneath (smegma), but this requires no attention.

The foreskin gradually loosens and can often be pulled back by 3 or 4 years of age, but sometimes not till much later. At this stage, the boy should be encouraged to pull it back and clean it in the bath.
