Dr Ajesh ISHRI
MBBS (Fiji), FRACGP (Aust), Master of Health Management (Aust), Master of Health Social Sciences (Aust), Master of Clinical Epidemiology (Aust). Performs minor surgery, experienced in the broad range of general practice to all age groups.

Dr Parvin ISHRI
MBBS, FRACGP (Aust), Frontline Management Boxhill Vic, Diploma in Child Health, Clinical Teaching, ACRRM.
Interests include women’s health, Child health, Dermatology, Chronic disease management, Preventative health, Weight loss management, Mental health, Men’s health, Minor surgery.

Dr Rashed AZIZ
MBBS (Bangladesh), FRACGP (Australia). Interests in General medicine, Geriatrics, Minor operations, Family and emergency medicine.

HOURS
Monday to Friday – 8 am – 5 pm
Saturday –  8.30am – 12noon

APPOINTMENTS
Appointments are necessary. Ring 4154 2466 for an appointment with your doctor of choice.
If you need a longer appointment, please tell the receptionist when you make your booking. Completion of Centrelink forms and employment medicals always need a longer appointment.
Urgent medical problems will always be dealt with promptly.

HOME VISITS
We can visit you at home only if you are too sick to come to the surgery if we have a Doctor available for the home visit. Please note this visit will be after hours when the Doctor completes their day and a fee will be involved as home visits are not bulk billed. If you require a home visit, please call the surgery as soon as possible after 8 am. If this is an URGENT situation call 000.

AFTER-HOURS
When the surgery is closed, You can visit Bundaberg Base Hospital Emergency Department 24hours completely Bulk Billed or The Friendlies Emergency Department, located at the Friendly Society Private Hospital Woondooma Street entrance, is open seven days a week from 8 am to 8 pm… This consultation cannot be Bulk Billed.
House Call Doctor is available Monday to Friday from 6 pm to 8 am with the call centre opening at 4 pm. Saturday the service is available from Noon with the call centre opening at 10 am and on Sunday and public holidays, the service is open 24 hours along with the call centre.
You can also call the After hours GP Helpline on 1800 022 222 for advice from 6pm-8am Mon to Sat, 12 noon Sat to 8 am Mon and 24 hours on public holidays. In QLD you can also access the helpline by calling 13HEALTH

Patient information and medical records are strictly confidential. Please talk to your doctor or the practice manager about any complaints or suggestions.

TELEPHONE CALLS
If you need to contact your doctor please phone the surgery during opening hours and leaving a message with a reception for the Doctor to action when able. Please note with reception if this is Urgent when they are taking your call. ONLY other Doctors and Specialist calls are put through.

FEES
Patients who hold a Medicare card are bulk-billed for all Medicare items. Exceptions include items such as insurance, pre-employment or commercial driving license medical examinations. Our surgery charges a $55.00 did not arrive fee if less than 2 hours notice is given to cancel the appointment.

Please bring your Medicare card and pension or concession card with you.
