Welcome to Synergy Medical Practice. We are and independently run General Practice located in St Leonards/Greenwich. We are a team of doctors and allied health professionals who strive to deliver your health care on a new level. We feel like every patient is part of the family and want to create an environment where our patients and staff feel safe and respected.

Our reception staff are here to support you.

We offer face to face, telehealth and phone consults.

We have a reminder system in place. If you do not want to receive these please let reception know.

APPOINTMENTS

Consultation is by appointment only. Please go to www.hotdoc.com.au to book an appointment online, or phone the surgery on the above number to arrange a time. If you need a longer appointment please let reception know on booking. Urgent appointments are available on the day if needed. If you do not arrive for your appointment a $35.00 non attendance fee is payable.

HOME VISITS

If you are unable to attend the surgery due to illness a home visit can be arranged.

Please phone the surgery as soon as possible or early on the day to arrange a time. Home visits are performed in the local area only in a 5km radius.

AFTER HOURS

For emergencies out of surgery hours please phone the after hours Home Doctor Service on 13 74 25. Instructions for contacting after hours doctors are also on the answering machine. Alternatively you can phone 000 for an ambulance.

TELEPHONE CALLS AND ­CORRESPONDENCE

The doctors are unable to take calls while consulting with patients, please leave a message with reception and the doctor will return your call when they are able to do so.

If you have an urgent issue please make an appointment to see the doctor.

Due to security reasons the doctor does not send or receive emails to or from patients, as normal email is not encrypted and therefore sensitive medical problems need a face to face consultation with the doctor.

PRESCRIPTIONS/REFERRALS/RESULTS

In most cases an appointment is required for repeat prescriptions, referrals or results.

CONSULTATION FEES

We are a private billing practice. Pension & DVA card holders and children aged 16 and under are exempt.

A list of our fees is on display in the waiting room.

Payment is required at the time of the consultation. We accept cash, cheque, Mastercard and Visa.

The complaints are received online, via email or mail.

PRIVACY

All patient records are kept strictly confidential. A copy of our privacy policy is available on request at reception.

For our records, we require the following personal information from all patients over the phone, as well as new patients when they come into their first appointment.

  • Full Name
  • Date of Birth
  • Address

If you are a new patient, our reception staff, nurse or doctors may also ask for the following details to inform your care:

  • Next of kin and emergency contact
  • Occupation
  • Religion
  • Cultural identity
  • Contact details, including phone numbers and an email address
  • Smoking and alcohol consumption
