Management of your Personal Health Information:
To be able to provide care, we collect information such as information to identify you, contact information and anything relevant to your medical care. This information will only be used to provide our patient with the best care we can offer.
Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. We abide by the ten National Privacy Principles available at http://www.privacy.gov.au/health/index.html.
Under the new Privacy Amendment (Private Sector) Act 2000, patients of our practice have the right to access their personal health information.
You have the right to access your Medical Records. However, due to the “medical jargon” used in the record, it is our policy that you will need to go through this with your Doctor to avoid any misunderstandings. Please speak to your Doctor if you have any concerns.
If you require your medical records to be forwarded to another practice, you will need to sign a consent form to release your information. Your new Doctor will have this available at reception for you to fill out and forward to us. There is an administrative fee involved depending on the size of the file. You will be advised of the fee before your records are released.
